By digitally signing below, I certify that the information I have given above is true and correct. I also verify that I have read, in full, all of the details below. It may seem like a lot, but it is extremely important as we take the care of any Winston's Wishes animal very seriously.

I acknowledge that Winston’s Wishes reserves the right to refuse my application to foster for any reason.
I agree to act as a volunteer foster home for Winston’s Wishes. I acknowledge and agree that activities performed by me as a volunteer will be performed strictly on a voluntary basis, without any pay, compensation or benefits. I agree to comply with the rules and regulations established by Winston’s Wishes and failure to do so may result in my immediate removal as a volunteer foster home.
I am aware of the nature of the activities to be performed as a volunteer foster home and I recognize and understand that there are certain risks inherent in handling animals and I accept those risks. I agree that all volunteer activities are to be performed at my own risk. I understand that if an accident or injury should occur, no matter how minor, I will notify Winston’s Wishes and seek any necessary medical attention utilizing my own medical insurance. On behalf of myself and my respective heirs and personal representative, I agree to indemnify and hold harmless Winston’s Wishes, its officers, directors, employees, agents and volunteers from and against any and all loss, damage, claims, liability, costs and expenses of any nature whatsoever, including but not limited to attorney's fees and disbursements, arising from or occasioned by my activities as a volunteer for Winston’s Wishes.
Winston’s Wishes makes no guarantee or representations about a selected animal's temperament or health, and will not be held liable for any injuries or bites or damages caused by the selected animal while in foster care.
Please understand the following about foster care volunteer assignments:
- Like most shelter animals, your foster may not be house/litter-trained. You understand that he/she may have accidents in your home.
- Shelter animals may expose you, your family and your pets to numerous diseases. There is always a risk, though usually minimal, of disease transmission.
- Like many dogs, your foster may chew/scratch on furniture, clothing, or other objects. You are comfortable working with this behavior and understand Winston’s Wishes is not responsible for these damages.
- You agree to keep your foster dog on a leash or enclosed fenced-in yard or home at all times. Your foster dog will never be outside of your home unsupervised.
- Representatives of Winston’s Wishes may need to contact or visit you to discuss the foster animal. You understand that you may be asked to complete evaluation forms on the animal. You agree to be entirely honest and forthright regarding the animal's behavior, be it positive or negative.
- All medical treatment will be done by Winston’s Wishes preferred vet, Bay View Veterinary Clinic in Mukwonago, unless there is an emergency. You understand you are not allowed to take your foster animal to the vet without our consent and will not be reimbursed for any bills. If there is an emergency situation, please reach out Winston’s Wishes immediately.
- You understand that your foster animal should not be transported to any location other than your residence or Winston’s Wishes without the permission of a Winston’s Wishes representative.
-Fostered animals are the property of Winston’s Wishes and can only be adopted or transferred through our organization.
I agree that Winston’s Wishes may use my image for Winston’s Wishes displays, educational programs and/or public relations, and I hereby release any such images or photographs for use in its programs, publications and purposes.
Volunteer Confidentiality Agreement:
As a condition of volunteering at Winston's Wishes, volunteers are expected to support Winston's Wishes mission and promote a positive image of Winston's Wishes to the public. As part of this responsibility, volunteers have an obligation to maintain the confidentiality of Winston's Wishes operations and various information to which they may have access. Therefore, in consideration of the opportunity to volunteer or to continue volunteering at Winston's Wishes, and as a necessary condition of same, you agree to the following statement:
I agree that while I volunteer at Winston’s Wishes, I will not disclose to any third party, use for my own personal benefit, directly or indirectly, any confidential information of Winston’s Wishes, except as required by law or with the express written consent of Winston’s Wishes.
I understand that should I violate this Agreement, I may be removed from my volunteer position. I understand that my violation of this Agreement may also subject me to civil legal action or criminal charges.
Volunteer Use of Online Forums:
In general, Winston’s Wishes respects the rights of volunteers to use online forums on their personal time as a medium of self-expression, and it is not the intent of Winston’s Wishes to police the online activity of its volunteers. However, Winston’s Wishes reserves the right to monitor and regulate volunteer postings/activities on any online forums when such conduct relates to Winston’s Wishes interests.
I understand that it is the policy of Winston’s Wishes to conduct criminal background checks of all individuals seeking to serve as volunteers. All information provided on this application is true and correct to the best of my knowledge. I understand that misrepresentations or omissions may be cause for rejection or may be cause for subsequent dismissal from the Winston’s Wishes volunteer program.
